PADI Open Water Diver Certification

A student diver must present the PADI Universal Referral Form for Open Water Diver Certification to a dive instructor. It states that the student diver has completed initial training with another dive training organization. This form instructs the instructor to complete the student’s open water dives and sign a statement. The form should then be sent to the original instructor who should certify it.

You can also use the Universal referral form for certification. However, PADI requires you to complete the course within 12 months of your last training. This is usually a two- or three day course. During this time, you must be able to dive with your buddy to a maximum depth of 60 feet/18 meters.

Medical statement

You will need to fill out the Medical Statement for Universal Reference Form if you want to become a scuba diving instructor. This document is vital and will make sure you are as healthy as you can before you dive. It is intended to ensure safety underwater. Please answer all questions honestly.

First, make sure your physician signs the form. This form must be signed by your physician before you begin training to become a scuba diver. A medical release is required for any new medical conditions. A photocopy of your certification card is recommended.

Logbook

Before a diver is allowed to dive open water, they must sign a medical form. Generally, this will be done before a certification course begins. A physician must sign the medical release. You will need a new release if your medical situation changes. PADI provides a medical release. The PADI Release of Liability/Assumption Of Risk/Non agency Acknowledgement For - General Training is the equivalent of the medical release.

The universal referral is different from that used in a diving logbook. It will list the open water dives and knowledge-development sections the student has completed. The instructor will sign the document certifying that the student has done the required dives. The student takes the completed form to his or her instructor and asks for certification.

Course completion requirements

The Universal Referral Program includes most of the well-respected scuba certification agencies. The Universal Referral Program means that a diver may take the theory or pool portion of an open water course with their local dive center and then do the open water check out dives with another agency. This can be accomplished with the assistance of a dive shop that participates in a universal referral program, such as SSI/NAUI/NASDSS/YMCA or any other organization.

PADI Open Water Diver courses require that the student passes the Knowledge Development and Open Water Dives sections. Before diving, the student needs to sign a medical release. A photo must be submitted by the student diver to accompany his/her certification card. Digital images are accepted.
