PADI Open Water Diver certification
A student diver must present the PADI Universal Referral Form for Open Water Diver Certification to a dive instructor. It states that the diver took initial training from another diver training organization. This form instructs the instructor to complete the student’s open water dives and sign a statement. The form should then be sent to the original instructor who should certify it.
Although you can use the Universal Referral Form to obtain the certification, PADI insists that you complete this course within twelve months from your last training. This is usually a two- or three day course. You will be required to be able dive with your buddy at a maximum depth to 60 feet/18 meters.

Statement for medical purposes
You will need to fill out the Medical Statement for Universal Reference Form if you want to become a scuba diving instructor. This important document will help you ensure your health before you dive. This document is designed to protect you underwater. Be sure to answer all questions honestly.
First, get your physician to sign the form. This form must be signed by your physician before you begin training to become a scuba diver. A medical release is required for any new medical conditions. Lastly, you should always take a photocopy of your certification card.
Logbook
A medical declaration is required before a diver may dive open water. This usually happens before certification classes begin. A doctor must sign the medical release. An updated medical release is necessary for patients with changing medical conditions. PADI offers a medical release. The medical release is equivalent to the PADI Release of Liability/Assumption of Risk/Non-agency Acknowledgement Form - General Training or the EU version.
The universal referral is different from that used in a diving logbook. The student will be able to list all open water dives completed and the knowledge development sections. It also contains a signed statement from the instructor confirming that the diver has taken the required dives. The student should then return this form to the original instructor who will be able to certify them as certified divers.

Course completion requirements
The Universal Referral Program is used by most well-known scuba certification organizations. This allows a diver to complete both the theory and pool parts of the open-water course at his or her local dive centre, while also completing the open-water check-out dives at another agency. This is done with the cooperation of a dive center with a universal referral program such as SSI, NAUI, NASDS, YMCA, or other organizations.
PADI Open Water Diver certification requires that students pass both the Knowledge Development section and Open Water Dives section. A medical release must be signed by the student diver before he or she can dive. To accompany their certification card, the student diver must also submit a photograph. Digital images can be accepted.
