PADI Open Water Diver Certification
A student diver must present the PADI Universal Referral Form for Open Water Diver Certification to a dive instructor. It states that the diver took initial training from another diver training organization. It directs the dive instructor to complete open water dives for the student, sign a certification statement, and then send the form to their original instructor, who should certificate the diver.
The Universal referral form can be used to get certification. But, PADI demands that you complete the course within 12 month of your last training. Usually, this is a two or three-day course. During this time you will need to be able to dive together to a maximum depth 60 feet/18m.

Statement of medical necessity
You will need to fill out the Medical Statement for Universal Reference Form if you want to become a scuba diving instructor. This important document will help you ensure your health before you dive. This document is meant to help you stay safe while diving, so be sure to answer every question honestly.
First, verify that your physician signs the forms. This form must be signed by your physician before you begin training to become a scuba diver. You will need a new medical release if you have any medical conditions. Finally, it is a good idea to always have a photocopy on hand of your certificate card.
Logbook
A medical statement must be completed before a diver can go open water. This is usually done before certification courses begin. A physician must sign the authorization. You will need a new release if your medical situation changes. PADI can provide a medical release. The medical release is equivalent to the PADI Release of Liability/Assumption of Risk/Non-agency Acknowledgement Form - General Training or the EU version.
The universal referral forms are different from those used in a dive logbook. It will list the open water dives and knowledge-development sections the student has completed. The instructor will sign the document certifying that the student has done the required dives. The student takes the completed form to his or her instructor and asks for certification.

Requirements for course completion
Most well-known scuba certification agencies participate in the Universal Referral Program. The Universal Referral Program means that a diver may take the theory or pool portion of an open water course with their local dive center and then do the open water check out dives with another agency. This is possible with the help of a diving center that participates in a universal program such as SSI and NAUI or NASDS.
PADI Open Water Divers requires that students pass the Knowledge Development as well as Open Water Dives sections. A medical release must be signed by the student diver before he or she can dive. A photo must be submitted by the student diver to accompany his/her certification card. Digital images are acceptable.
